What to Expect For An Exam

For First-Time Patients


Please bring the following items:

  • Proof of Identification/ID Card

  • Medical Insurance Card

  • Covid-19 Vaccination Card

  • A list of any Medications, Vitamins, and Supplements that are currently being taken, include the amount of mcg/IU (Bring Bottles or Photos)

  • If you wear contact lenses or glasses: Bring in Contact Lenses' foils and/or Eyeglasses


For Returning Patients


​​​​​​​Please bring the following items:

  • Medical Insurance Card: For those who have changed their insurance or have received their latest card

  • Covid-19 Vaccination Card: If you have received the latest booster, please notify us about it

  • A list of any new Medications, Vitamins, and Supplements that are currently being taken, include the amount of mcg/IU: Bring Bottles or Photos

  • If you are instructed to or need to: Bring or Wear your contacts and/or glasses
